This policy and your child's health information
If your child receives care from Children's Village PPEC, how we use and protect health information is described in our Notice of Privacy Practices: [To be provided: link to the Notice of Privacy Practices]. Where this policy and that notice differ about health information, the Notice of Privacy Practices controls.
Information you give us
We collect only the information you choose to send through our forms:
- Schedule a Visit: your name, phone number, and email address and, if you choose to share them, your preferred language, your child's age, your county, how you prefer to be contacted, how you heard about us, and your message.
- Contact: your name and email address and, if you choose, your phone number, whether you are a parent or a professional, and your message.
- Refer a Family: your name, phone number, and email address and, if you choose, your organization, your role, the family's county, and your notes.
- Consent: when you submit a form, we record that you checked the consent box, the wording you agreed to, and the date and time.
Please don't send medical information
Please do not include diagnoses, medical history, or other health details in a website form. Our team will follow up and collect what is needed through an appropriate channel.
Our forms are not monitored around the clock and must not be used for emergencies. In an emergency, call 911.
Information collected automatically
This website does not use cookies, analytics, advertising tags, tracking pixels, or social media plugins, and it does not build a profile of your visit. See our Cookie Policy.
Like any website, the servers of our hosting provider receive technical information when a page loads, such as your IP address, browser type, the page requested, and the time. This is used to deliver the site, keep it secure, and fix problems. To protect our forms from spam, we briefly use your IP address to limit repeated submissions; it is not stored with your submission.
How we use information
- To respond to your message, schedule a visit, or follow up on a referral.
- To contact you by phone, email, or text message about your request, as you agreed when you submitted the form.
- To operate, secure, and improve this website.
- To meet our legal, regulatory, and licensing obligations.
We do not sell or rent personal information, we do not use it for targeted advertising, and we do not share it with others for their own marketing.
Text messages
If you ask us to contact you by text message, we will text you only about your request. Message frequency varies. Message and data rates may apply. Reply STOP to stop receiving texts, or HELP for help. Mobile numbers and text-message consent are never shared with third parties for marketing.
Who we share information with
- Service providers that host this website and deliver form submissions to our team. They may use the information only to provide those services to us.
- Government authorities or others when the law requires it, or to protect the rights, safety, and security of children, families, staff, or the public.
- A successor organization if Children's Village PPEC is part of a merger, acquisition, or sale of assets, subject to this policy.
- Anyone else only with your permission.
How long we keep information
We keep form submissions only as long as needed to respond to you and to keep reasonable business records, and then we delete them. If your child enrolls, care records are kept as the law requires and as described in our Notice of Privacy Practices.
How we protect information
Information you send through our forms is encrypted in transit (HTTPS). We use reasonable administrative, technical, and physical safeguards. No method of sending or storing information is completely secure, so we cannot guarantee absolute security.
Children's privacy
This website is meant for parents, caregivers, and professionals. It is not directed to children, and we do not knowingly collect personal information from children under 13. The optional child's-age field asks only for an age.
Your choices
You can ask us what information we have from your form submissions, ask us to correct or delete it, and stop texts or emails at any time. Contact us at [To be provided: email address] or [To be provided: phone number]. We may need to confirm your identity first, and we may keep information when the law requires it.
